2020 Fellow of Pediatric Perfusion Interest Form

Thank you for your interest in being considered for the Fellow of Pediatric Perfusion designation for the 2020 application period. Please fill out the following information. You must answer all questions prior to beginning the formal application. If the form is incomplete the application will not be considered for review. Please note that completed applications with all application materials are due to AmSECT no later than December 15, 2019. All application requirements must be submitted by December 15 or your application will not be reviewed. 

MEMBERSHIP REQUIREMENT
You must be a current member in good standing to be considered for and retain the Fellow of Pediatric Perfusion designation. Your membership must not expire prior to March 2020. The Fellow of Pediatric Perfusion (FPP) is an honor bestowed by AmSECT and membership is a prerequisite to apply. For those awarded the FPP designation, membership in AmSECT must be maintained to use the designation. In the event the FPP’s AmSECT membership lapses, reapplication shall be required for reinstatement of the FPP designation. Retiring FPPs will be awarded FPP Emeritus status upon written request of the FPP approval committee. The FFP Emeritus designation does not require ongoing AmSECT membership. A completed formal application is required from each applicant. Forms will be available for download after the applicant completes the Request for Fellow of Pediatric Perfusion Application form. 

CASE REQUIREMENT
You must be able to document two hundred (200) CPB cases for correction or palliation of congenital heart
disease

  • Eighty or more patients included in the 200 should weigh 10 kg or less
  • Maximum of forty cases of the 200 may be as the backup perfusionist.

ACTIVITY IN THE FIELD OF PEDIATRIC PERFUSION REQUIREMENT
You will be asked to include a demonstrated a history of interest in pursuing professional excellence that may include the following:

  •  Membership in local and national perfusion societies and organizations 
  • Teaching participation in local, regional or national programs 
  • Continuing education by attendance at professional meetings, courses and seminars

AmSECT Membership Eligibility

An Active Member shall be any perfusionist active in the practice of extracorporeal circulation technology. There are also opportunities to become a Transitional Active Member for those less than a year removed from graduation from an approved accredited training program. Other options include; an Associate Membership, International Membership, Perioperative Blood Management Clinician Membership, and Student Membership.
