With the beginning of the new year I will be presenting PerfShareLive sessions again monthly. The discussion sessions will last 30 minutes. A Table top scenario discusses a CPB or ECMO failure, the effects of the failure and the causes of the failure. It also discusses the pre-emptive management actions that should be taken to prevent the failure from occurring or, if the failure does occur, the management actions that can be taken to mitigate the failure. The risk is quantified along with ways to reduce the failure risk.

Session 1: “Goal Directed Therapy/Perfusion and Oxygen Pressure Field Theory: What's The Connection?”

Goal directed therapy and perfusion strives to meet the minimal oxygen delivery needed to prevent organ failure and promote healing. This can be done by calculating oxygen delivery as well as the CO2 production to O2 delivery ratio. If a patient is on pump this is done by controlling blood flow, hematocrit, SVO2 and some other variables such as temperature and pressure. Those are all good things to do, but they miss the point. The point is to achieve a safe level of tissue oxygenation and tissue CO2 to prevent patchy areas of anoxia and hypercapnea in tissues at the capillary level; i.e., OPFT. Both the tissue O2 and tissue CO2 must be controlled to prevent acute organ failure. The most important variable in this regard is perfused capillary density which is difficult to measure directly but can be measured indirectly.

Session 2: “Cardioplegia home brew or pickles in your chili!?”

Putting pickles in chili is not something that is wise to do; like perfusionists mixing their own cardioplegia and other IV solutions and medications. Just what is compounding and are perfusionists and ECMO specialists qualified to do it? There is virtually nothing about this in the AmSECT Standards and Guidelines, demonstrating just how far out of the loop the perfusion profession really is. There are federal regulations concerning compounding that individual perfusionists or perfusion programs might be violating. We will be joined by Thomas N Muziani PA-C, CP, President/ CEO of Hemo-Stat Blood Management Consulting and Christopher Lomax, Pharm D., Sr. Marketing Manager, Pharmacy Advisor, Braun Medical Inc.

Session 1: “Table Top Scenario: ECMO venous blood line jerking a/k/a chugging.” Venous blood line chugging is a simple problem with many possible causes. This might be “old hat” to an experienced perfusionist or ECMO Specialist. But to perfusion students and ECMO Specialists who are new to the game or who do not participate in ECMO procedures very often, this will be a good review. Two presentations will be made: March 22 Thursday at 1830 central daylight savings time and repeated on March 23 Friday at 0730 central daylight savings time. Session 2: “Table Top Scenario: Inadequate venous drainage caused by non- anatomical blood line obstruction during CPB.” Like chugging of the venous blood line on an ECMO pump, this is a problem well known by experienced perfusionists. But to perfusion students or inexperienced perfusionist, this will be a good review that should allow them to quickly evaluate and resolve the problem without looking like a “rookie”. AmSECT Membership Eligibility

An Active Member shall be any perfusionist active in the practice of extracorporeal circulation technology. There are also opportunities to become a Transitional Active Member for those less than a year removed from graduation from an approved accredited training program. Other options include; an Associate Membership, International Membership, Perioperative Blood Management Clinician Membership, and Student Membership.
