Note: Even if you are only changing one item, e.g., your user name but not your password, make sure to enter choices for both. The National Office will notify you by e-mail to verify that your new choice for a user name and password has been approved. If you do no have e-mail, please indicate None in the space provided for e-mail. A postcard will be mailed to you verifying your new choices for a user name and password.
